7/15/25 -

• Dante met with the Ryan White team: Aubree Buccino, Lizbeth Vazquez, Sandra Perez Ruiz, Paola Monge, and Julyeishka Colon.
• This meeting had 3 objectives: Follow-up on the Mind Matters Training, remind the site about the Performance Assessment Survey due on July 25th, and plan which Action Item on the plan they wanted to address.
• It was learned during the session that Kasey Harding, who oversees CHC is responsible for completing all the RW surveys, so Maria Lorenzo (who popped in) confirmed she would request the survey be completed ASAP.
• In addressing on-going check-ins on the mental health of the staff, Dante offered to create an anonymous provider check-in survey in Qualtrics or meet 1-on-1 with staff for a 15-minute session. Aubree and Dante established a follow-up meeting to take place for 30-minutes on July 29th to discuss what the group decided.
• The group decided to move forward with addressing A2 on their Action Plan: Undetectable Status/Med Compliance of All Patients. Dante explained once he received the necessary information from the Performance Measures Survey that he would present the findings at the next group's coaching session on August 15th and discuss with the team next steps.
