Mission Statement: Create a system of local governance that is coordinated, collaborative, transparent, effective, responsive and creative and that fulfills the HRSA goal of providing a comprehensive continuum of high quality, community based care for PLWHA.
Under the Lead Agency model, there will be a monthly HIV Care Continuum meeting between the Lead and the subcontractors. These working meetings will provide the basis for monitoring service delivery based on CAREWARE-generated data including but not limited to: service utilization, number of new and established clients, cost/client and cost/service. This data will be analyzed in real-time to inform the Continuum whether or not service targets are being met, in particular, with regard to the 6 special populations. Importantly, these meetings will enable us to better define service priorities; enhancing meaningful inter-agency collaboration and sharing of limited resources; definite the appropriate balance between community-based and clinic-based treatment models. The meetings will help us to standardize our reporting based on recommendations from the Part A office; refine our outcome measures and ultimately to define uniformly acceptable guidelines for using these measures to determine resource allocation. Finally, this collaboration will also help to plan strategically to find alternative funding sources for PLWHA in our Region.
